PEDIATRIC HOME CARE SERVICES

Free in home nursing services for medically fragile children under the age of 21. GAPP (Georgia Pediatric Program) is a Medicaid program that provides services to medically fragile children ages 0-21 at no cost to the family. Determination is based on the child’s medical needs. Families who are eligible for Medicaid can apply for GAPP at any time. 

Step 1


Meet with Capstone Quality Care to sign consent paperwork and gather other required information such as letters of medical necessity from physicians, hospitalization discharges, and more. 

Step 2


Capstone Quality Care prepares and submits paperwork to GAPP. GAPP may take up to 30 days to make a decision (approve or denial). 

Step 3


Capstone Quality Care receives decision from GAPP.

Step 4


Capstone Quality Care contacts you to schedule the start of care or to discuss the appeal process in case of denial.

COVID-19


WE'RE HERE FOR YOU

Capstone Quality Care staff are provided education and training on COVID-19 infection control practices including the importance of frequent handwashing, cleaning and disinfecting surfaces regularly, and proper use of personal protective equipment (PPE). Caregivers and nurses are provided with proper PPE for use in all clients’ homes, wear masks while providing care in the home, and practice social distancing whenever possible.


Our staff are screened for COVID-19 symptoms prior to reporting to work. Our staff that are experiencing symptoms or are living with sick household members are required to refrain from reporting to work and must follow any applicable isolation and testing requirements before returning to work.
